Please complete all pages and client signature spaces within this document and return prior to your first appointment. If you prefer, you may plan to arrive 15 minutes prior to your scheduled appointment to complete them ahead of time.

Please complete and sign this intake form prior to your first visit. Your initial visit will consist of about 15 minutes to review your current needs and the history in this packet in order to best plan your service. The remaining 45 minutes or so will consist of hands-on time. After your first session, further sessions may not require the full 15 minutes of time to prepare, depending on any updates you'd like to discuss.

BUSINESS/FACILITY COLLABORATION AGREEMENT

Do the employees at your business, school, union facility, or warehouse need a little self-care? This form gives basic information on what to expect, and will be used to finalize the agreement for booking.
